Breast implants add volume to existing breast tissue to create a fuller chest. After implantation, however, the shape of the breast is not always natural; indeed, breasts with implants often appear rounded and lose the pleasant conical shape that is typical of young breasts. Moreover, the areolae broaden and the subcutaneous areolar tissues become thinner. If the silicone implants are heavy, they may detach from the chest wall, creating the appearance of a “ball in a sock”. If the implant is not small, it may become ptosic within a few years. Today, these defects can be corrected by means of an innovative ambulatory procedure that is carried out under local anesthesia: Conization of breasts that have silicone implants by means of the elastic thread and Jano needle. Designed by the plastic and reconstructive surgeon Sergio Capurro, this procedure was published in the Medical Video Journal CRPUB.ORG in December 2017 as a sequel to the previously published the 3-D animation. The procedure enables breasts that have implants to be conized through a 1 cm incision in the areolar margin. Through the same access, the areola is reduced and the apex of the breast is conized. Moreover, through a 3-mm incision in the anterior axilla, the breasts can be raised and drooping corrected. The results are permanent. This is the first procedure that enables breasts to be permanently lifted by means of threads. This has been made possible by the invention of an impalpable elastic thread that is transformed into a “natural” ligament and by the technique of anchoring the thread to the subcutaneous thoracic tissue by means of the two-tipped needle.
